Visa, MasterCard, Discover, American Express, money orders, personal checks (for in-office only), cashier’s checks and cash. For Surgery: Personal checks are only accepted if paid two (2) weeks prior. No post-dated checks will be accepted. There is a $30.00 charge associated with any returned check.
We are pleased to accept CareCredit. Please keep in mind that CareCredit is a credit card and is not an in-house credit program. CareCredit is not an interest-free credit card, although they do offer several deferred interest plans. Our office staff will be happy to discuss CareCredit with you at your consultation.

Apply for CareCredit™
CareCredit requires that you have two (2) valid forms of identification when applying for CareCredit. They also require two (2) valid forms when using your account prior to receiving your card in the mail. The lists below are approved forms of identification:
Primary ID (must be valid):
- State issued Driver’s License
- State issued I.D. Card
- Military ID
- Government Issued Green Card/Resident Alien Card
Secondary ID (must be valid):
- American Express
- Department Store Credit Card
- Gas Company Credit Card
Please note that CareCredit requires a valid Primary ID when using your CareCredit card.
